Surgical Abortion
The surgical abortion procedure (also called suction D &. C and many other names) has been performed for many years. This procedure is appropriate for pregnancies dating from very early to about thirteen weeks, or the end of the first trimester. It is extremely safe and is a relatively short procedure. There are many variations on this procedure, even within our practice, but this description covers the basic aspects.
The Procedure
After the dating ultrasound, an individualized discussion and a review of the consent form will occur prior to the procedure itself. The physician performs a bimanual exam to determine the uterine position. The speculum is then placed to visualize the cervix.
A local anesthetic (paracervical block) is placed into the cervix. This is intended to modify the discomfort of the procedure. This block only takes a few seconds and does not take away all of the discomfort. However, it does enable many women to tolerate this relatively short procedure. Please refer to IV sedation and general anesthesia as options for pain control. The procedure will include dilation of the cervix followed by an evacuation of the uterus. This phase of the procedure usually takes a minute or two depending on the gestation. The procedure is then essentially over. An ultrasound exam may be performed immediately after the procedure to assure that the procedure is completed.
An antibiotic pill is given by mouth at the time of the procedure, and another dose provided to be taken as directed. The patient will rest and will be continually monitored by the clinician. A plan for contraception is discussed and provided. Discharge instructions are reviewed before the patient leaves our facility. A follow-up appointment is recommended within the next two weeks.
